Chicago AGD has arranged with ACES to have two CPR renewal courses for those who may need to renew their
BLS Provider licenses before Illinois Dental License renewal on September 30, 2018.
Course: American Heart Association BLS Provider Renewal
Dates: Friday, 5/25/2018 or Wednesday, 5/30/2018
Time: 9-11 AM
Location: 2030 E. Algonquin Rd., Suite 406 Schaumburg, Illinois
Cost: $41.00
Prerequisite: previously BLS certified

No CE given.
How to Register:
Go to ACES website: cardiosave.com, create a “User Profile” and when creating the user profile in the Employee ID No type: CADG406, once a profile is created, call ACES office at (847) 907-4768 and tell who ever answers the phone that you are
from Chicago Academy of General Dentistry, and the date you want to attend. They will gather the information and process the request. Payable with VISA or MasterCard.
